| Instructions to complete the Course of Study Form 1. Fill in the school year. 2. Include the child's and parent's full name. This is very important if there are different last names between children and parent. 3. Include the grade level for which your child is in. 4. Include the name of book or program under Book, Activity section. If you are taking a class at EIE then you can just mention the class and teacher. 5. Make any comments you wish in the Comments Section 6. Mail or bring completed forms to our Resource Center. 7. Just fill in the blanks that pertain to your child. You do not have to fill in all areas. |
